Investigators report that a vehicle with two occupants was traveling north in the left lane, while another vehicle with one occupant was in the right lane. The right-lane vehicle crossed over, sideswiped the vehicle in the left lane, then lost control, crossed the median, and slammed into a van with 15 occupants traveling southbound head-on.

In total, 18 people were involved in the crash. Four victims were killed, and 17 patients were transported for treatment.

Both directions of Highway 111 remain closed as THP continues its investigation. Putnam County units are assisting at the scene.
